Driving directions:

Just West of Applegate on Oregon Highway 238 find Thompson Creek Road (paved) heading south.
0.0 TURN SOUTH onto Thompson Creek Road
12.0 PAVEMENT ENDS, TURN HARD RIGHT onto Road 1005 uphill (not Road 1020)
12.3 BEAR LEFT
14.3 PASS Lower O'Brien Creek TH on left (camping along Road 300 on left)
16.0 PARK at Upper O'Brien Creek TH (road ends)
The road had significant erosion between the Lower and Upper O'Brien Creek trailheads. I recommend a high clearance vehicle if driving.

Hiking directions:

I followed the trail west to a junction at about 5540 feet elevation just after crossing O'Brien Creek. Here I turned right and began switchbacking up the head of the valley. At about 6200 feet elevation I reached the Boundary Trail and turned right towards Windy Gap.

About half-way to Windy Gap from that junction, at about 6300 feet, I left the trail and climbed directly up open-forested and meadow slopes to the north ridge of Grayback Mountain. I followed the ridge south over some firm snow and generally left of the ridge top to the summit. I returned the same route.
